The Mouth and Heart Connection

Cardiovascular disease is a class of disease that affects the heart and/or blood vessels. It is estimated that more than 80 million people in the United States have one or more forms of cardiovascular disease; these forms include high blood pressure, coronary heart disease (acute heart attack and angina pectoris), stroke, and heart failure. Studies have shown that there is a link between cardiovascular disease and periodontal (gum) disease, the chronic inflammation and infection of the gums and surrounding tissue. Forms of gum disease, such as gingivitis (gum inflammation) and periodontitis (bone loss), can be indicators for cardiovascular problems, which is why it is important for individuals at risk for cardiovascular disease to visit a dentist on a regular basis, practice good oral hygiene, and keep their dentist informed of any oral and overall health issues.

How are periodontal disease and cardiovascular disease connected?

It has been suggested that the inflammatory proteins and bacteria associated with gum disease enter a person’s bloodstream and can cause various effects on the cardiovascular system. A study published in the February 2005 issue of Circulation examined the presence of the bacteria known to cause periodontitis and the thickening of the blood vessel wall typically seen in heart disease. After examining samples from more than 650 participants, the investigators concluded that the presence of the same bacteria known to cause periodontitis was associated with an increased level of blood vessel thickening.

What can I do to keep my gums and heart healthy?

Practicing proper oral hygiene is essential to maintaining healthy gums. This includes flossing regularly, brushing twice a day with antibacterial toothpaste, and visiting a dentist at least every six months. A healthy diet and regular exercise can help improve both your cardiovascular health and your overall health.

What do my physician and dentist need to know?

It is important to keep all medical professionals up-to-date on your oral and overall health issues. Inform your physician if you have been diagnosed with a form of periodontal disease or are experiencing any issues with gum inflammation. Likewise, inform your dentist if you have been diagnosed with any form of cardiovascular disease, have experienced any cardiovascular problems, or have a family history of cardiovascular disease.

What other risk factors are associated with cardiovascular disease?

Individuals who are most at risk for cardiovascular disease include those over age 65, African-Americans, Hispanics, and males. While these particular factors cannot be changed, there are some risk factors that you can change through lifestyle management and/or medical treatment to reduce your risk for cardiovascular disease. These risk factors include smoking, high cholesterol, high blood pressure, physical inactivity, obesity, excessive alcohol consumption, and stress. Questions? Be sure to ask your physician and dentist.

Flossing

Do I really need to floss?

Yes. Floss removes plaque and debris that sticks to teeth and gums in between teeth, polishes tooth surfaces, and controls bad breath. Flossing is the single most important weapon against plaque and, in some ways, more important than the toothbrush. By flossing your teeth daily, you increase the chance of keeping them for a lifetime and decrease the chance of getting gum disease. Many people don’t spend enough time flossing and many never have been taught the proper way to floss. Floss at least once a day and spend at least two or three minutes.

Which type of floss is the best?

Dental floss comes in many forms: waxed and unwaxed, flavored and unflavored, wide and regular. They all clean and remove plaque. Wide floss, also known as dental tape, may be helpful for people with a lot of bridgework and is usually recommended when the spaces between teeth are wide. Waxed floss might be easier to slide between tight teeth with very little space between. The unwaxed floss, however, makes a squeaking sound to let you know your teeth are clean. Bonded unwaxed floss does not fray as easily as regular unwaxed floss, but does tear more than waxed floss.

How should I floss?

The spool method: Take an 18-inch piece of floss and wind the bulk of the floss lightly around the middle finger. (Don’t cut off your finger’s circulation!) Wind the rest of the floss similarly around the same finger of the opposite hand. This finger takes up the floss as it becomes unusable. Maneuver the floss between teeth with your index fingers and thumbs. Bring the floss up and down several times forming a “C” shape around the tooth being sure to go below the gum line.

The loop method: This method is suited for children or adults with less nimble hands, poor muscular coordination or arthritis. Take an 18-inch piece of floss and make it into a circle. Tie it securely with three knots. Place all of the fingers, except the thumb, within the loop. Use your index fingers to guide the floss through the lower teeth, and use your thumbs to guide the floss through the upper teeth, going below the gum line forming a “C” on the side of the tooth.

Do I need a waterpick?

Waterpicks should not be used as a substitute for brushing and flossing. They are effective, however, around orthodontic braces that retain food in areas a toothbrush cannot reach. Unlike flossing, waterpicks do not remove plaque. Dentists frequently recommend waterpicks for people with gum disease.

Bruxism

What causes bruxism?

Bruxism can have numerous causes, such as bite problems, stress, medical conditions, or certain medications.

What are signs of bruxism?

Most people with bruxism are not aware of the condition, and only approximately 5 percent develop symptoms (such as jaw pain and headaches) that require treatment. In many cases, a sleeping partner or parent will notice the bruxism before the person experiencing the problem is even aware of it. The noise resulting from bruxism can be quite loud.

Bruxism can result in abnormal wear patterns on the top surfaces of teeth, unusually sensitive teeth, notching of the teeth at the gumlines, as well as severe damage to the teeth, including fractures. Bruxism also is a significant cause of tooth loss, gum recession, and loosening of the teeth.

What are the symptoms of bruxism?

The symptoms of bruxism vary and can include anxiety, stress, and tension; depression; earache; eating disorders; headache; insomnia; and a sore or painful jaw. If left untreated, bruxism eventually shortens and blunts the teeth being ground and can lead to facial muscle pain and TMD. In severe chronic cases, it can lead to arthritis of the temporomandibular joints.

How is bruxism diagnosed?

The patient often becomes aware of the condition during a routine dental examination. Your dentist will be able to recognize the signs of bruxism during a dental exam and may even suggest further analysis of your bruxism, such as recommending an overnight stay at a sleep laboratory.

How is bruxism treated?

There is not always a definitive cure for bruxism, but the signs and symptoms can be reduced or eliminated through dental treatment. Treatments can include mouthguards, bite adjustments, biofeedback devices, and repair of damaged teeth.

Do you have questions about bruxism?

Talk to your dentist.

Dr. Jerrold Goldsmith, DDS, FAGD is a comprehensive GENERAL and COSMETIC DENTIST who has been serving the Indianapolis, Indiana community for over 25 years.
FACT #2:The average male in the 12- to 19-year age group drinks the equivalent of 868 cans a year.

Should I limit drinking soda?

YES! Whether at school, home or on the weekends, teens are drinking more soda than they have in the past. In 1977, 12 to 19-year-olds drank 16 ounces of soda a day. In 1996, this same age group consumed an average of 28 ounces a day. Not only is sugar harmful to teeth, acidic flavor additives can also erode and damage tooth enamel. There are simple ways you can limit the harmful effects of sodas. Try sipping soda through a straw. It cuts down on the contact the beverage has with your teeth. Rinse your mouth with water after drinking soda. It can also reduce the risk of cavities.

Why should I avoid oral piercings?

Tongue piercing remains a teen trend, however it is not always a healthy choice for your mouth. People chip teeth on tongue piercings while eating, sleeping, talking and chewing on the jewelry. Tongue piercing commonly causes fractured teeth. The fracture can be confined to tooth enamel and require a filling, or it may go deeper; in which case, can cause a need for a root canal or extraction.

Infections are also common with oral piercings, and they cause more than pain. A tongue can swell after being punctured, however in some cases the tongue becomes infected and swells so much that it may cut off breathing. Unclean piercing equipment can cause other infections, such as bloodborne hepatitis.

Why should I make time for healthy habits?

Quick meals in the form of “nutrition” bars and fast food help keep you alert and on schedule between school, extracurricular activities and part-time jobs. However, today’s fast-paced lifestyle threatens to leave the teen generation with permanent damage to oral and overall health. You can keep travel-size brushes in lockers or back packs. Chewing sugarless gum with xylitol after meals or snacks can also help cleanse your mouth. Drinking water throughout the day can help clean your teeth of excess bacteria and food debris. Teens should be sure to see their dentist at least twice a year. Regular dental visits can help catch minor problems before they become major ones!

Source: Academy of General Dentistry (www.agd.org)

Eating disorders are real, complex, and often devastating conditions that can have serious consequences on your overall health and oral health. Telltale early signs of eating disorders often appear in and around the mouth. A dentist may be the first person to notice the symptoms of an eating disorder and to encourage his or her patient to get help.

What are the different types of eating disorders?

Anorexia nervosa is a serious, potentially life-threatening eating disorder characterized by self-starvation and excessive weight loss.

Bulimia nervosa is a serious, potentially life-threatening eating disorder characterized by a cycle of bingeing and compensatory behaviors (i.e., self-induced vomiting, use of laxatives, diuretics, or enemas) designed to undo or compensate for the effects of binge eating.

Binge eating is an eating disorder characterized by recurrent episodes of uncontrollable eating without the regular use of compensatory measures to counter the effects of excessive eating. Binge eating may occur on its own or in the context of other eating disorders.

Pica is an eating disorder that is described as “the hunger or craving for non-food substances.” It involves a person persistently mouthing and/or ingesting non-nutritive substances (i.e., coal, laundry starch, plaster, pencil erasers, and so forth) for at least a period of one month at an age when this behavior is considered developmentally inappropriate.

How do eating disorders affect health?

Eating disorders can rob the body of adequate minerals, vitamins, proteins, and other nutrients needed for good health. Without the proper nutrition, gums and other soft tissue inside your mouth may bleed easily.

Frequent vomiting may affect teeth as well. Repeated exposure to strong stomach acid can cause much of a tooth’s outer covering (enamel) to be lost, resulting in changes to the teeth’s color, shape, and length. The edges of teeth may become thin and break off easily. Eating hot or cold foods or drinks may become uncomfortable. Repeated vomiting also can cause mouth sores, dry mouth, cracked lips, and bad breath.

How do dentists detect eating disorders?

Changes in the mouth are often the first physical signs of an eating disorder. Bad breath, sensitive teeth, and tooth erosion are just a few of the signs that may indicate whether a patient might be suffering from an eating disorder. A dentist who determines that a patient has an eating disorder can refer the patient to the appropriate medical personnel, as well as teachthe patient about how to minimize the oral effects of the eating disorder.

Can a person recover from an eating disorder?

Yes. Professional help, nutritional counseling, and having a good support network all play a crucial role in the recovery process. For more information, talk to your dentist or physician.

Source: Academy of General Dentistry (www.agd.org)

It is estimated that up to 20 million people have diabetes, but only two-thirds of these individuals are diagnosed. Studies have shown that diabetics are more susceptible to the development of oral infections and periodontal (gum) disease than those who do not have diabetes. This relationship causes great concern because serious gum disease may have the potential to affect blood sugar control and contribute to the progression of diabetes. That’s why it’s important for people with diabetes to visit a dentist on a regular basis and to keep the dentist up to date on the status of the diabetic’s oral and overall health.

How are gum disease and diabetes related?

Because diabetes reduces the body’s resistance to infection, the gums are at risk for gingivitis, a reversible form of gum disease usually caused by the presence of bacteria. These bacteria produce toxins that create a sticky film that accumulates on teeth, both above and below the gum line, leading to inflammation. If left untreated, gingivitis can progress to periodontitis, an irreversible destruction of the tissues that surround and support the teeth.

What other problems are associated with diabetes?

Other oral problems associated with diabetes include thrush (candidiasis), an infection caused by a fungus that grows in the mouth, and dry mouth, which can cause soreness, ulcers, infections, and cavities. To prevent problems with bacterial infections in the mouth, your dentist may prescribe antibiotics, medicated mouth rinses, and more frequent cleanings.

How can I stay healthy?

Brush your teeth with an antimicrobial toothpaste containing flouride and rinse with antimicrobial mouthwash at least two times a day. People with diabetes who receive good dental care and have good insulin control typically have a better chance of avoiding gum disease.

To improve their quality of life and their oral health, people with diabetes need to pay close attention to diet and exercise. People with diabetes should be sure that both their medical and dental care providers are aware of their medical history and periodontal status. To keep teeth and gums strong, those with diabetes should be aware of their blood sugar levels in addition to having their triglycerides and cholesterol levels checked on a regular basis.

What is the best time to receive dental care?

If your blood sugar is not under control, talk with both your dentist and physician about receiving elective dental care. Types of dental procedures and appointment length are dependent on the level of diabetic control. Also, schedule morning appointments because blood glucose levels tend to be more stable at this time of day. If you have a scheduled appointment, eat and take your medications as directed. See your dentist on a regular basis, keep him or her informed of your health status.

Source: Academy of General Dentistry (www.agd.org)

June is National Men’s Health Month. Across the country, preventative screenings, health fairs, media spotlights, and other health education and outreach activities will raise the awareness of preventable health problems and encourage early detection and treatment of disease among men and boys.

Why is oral health important for me?

Men are less likely than women to take care of their physical health and, according to research, their oral health is equally ignored. Good oral health has been linked to longevity, yet one of the most common factors associated with infrequent dental checkups is just being male. Men are less likely than women to seek preventive dental care and often neglect their oral health for years, visiting a dentist only when a problem arises. In fact, when it comes to oral health, the average man brushes his teeth 1.9 times a day and will lose 5.4 teeth by age 72. If he smokes, he can plan on losing 12 teeth by age 72. Men also are more likely than women to develop oral and throat cancer and periodontal (gum) disease.

Do you take medications?

Since men are more likely to suffer from heart attacks, they also are more likely to be on medications that can cause dry mouth. If you take heart or blood pressure medication or if you take antidepressants, your salivary flow could be inhibited, increasing the risk for cavities. Saliva helps to reduce the cavity-causing bacteria found in your mouth.

Do you use tobacco?

If you smoke or chew tobacco, you have a greater risk for gum disease and oral cancer. Men are affected twice as often as women, and 95 percent of oral cancers occur in those ages 40 and older. More than 8,000 people die each year from oral and pharyngeal diseases. If you use tobacco, it is important to see a dentist frequently for cleanings and to ensure your mouth remains healthy. Your general dentist can perform a thorough screening for oral cancer and help with tobacco cessation.

Do you play sports?

If you participate in sports, you have a greater potential for trauma to your mouth and teeth. If you play contact sports or participate in any activity with potentail for face injury, it is important to use a mouthguard, which is a flexible appliance made of plastic that protects teeth from trauma. Your dentist can discuss mouthguard options with you.

How often should I visit a dentist?

Regardless of gender, it is important to visit a dentist twice a year. Inform your dentist of any medications you are taking, as well as tobacco use and sports activities. In between visits, floss daily and brush your teeth with fluoridated toothpaste. For more information on men’s oral health and other oral health topics, visit www.knowyourteeth.com.

Good Oral Health Starts at Home

Maintaining a healthy smile for a lifetime starts early. That’s why it’s important for you and your family to visit your general dentist regularly and practice good oral hygiene at home.

Why is brushing so important?

Regular toothbrushing with toothpaste plays a major role in reducing the growth of plaque—a thin, sticky film of bacteria that creates cavities, tooth decay, and gum disease. When you brush your teeth, you remove most of the plaque-causing bacteria. Brushing also helps clean and polish your teeth, removes stains, freshens your breath, and leaves your mouth feeling clean.

What is the best technique for brushing?

There are a number of effective brushing techniques. Patients are advised to check with their dentist or hygienist to determine which technique is best for them, since tooth position and gum condition vary. One effective, easy-to-remember technique involves using a circular or elliptical motion to brush a couple of teeth at a time, gradually covering the entire mouth. Avoid using a back-and-forth motion, because this can cause the gum surface to recede, can expose the root surface, or make the root surface tender.

How long should I brush?

It is recommended that you spend at least two minutes brushing your teeth at least twice a day. If you think you’re already doing a good job, you might be surprised: Patients generally think they are brushing longer than they are—most spend less than one minute brushing. To make sure you are brushing for the full two minutes, set an egg timer or invest in a toothbrush with a built-in timer. And, be sure to spend the time wisely: Brush the front and back of teeth, the tongue, the chewing surfaces, and between teeth.

How can I get my children to brush?

The best way to guide your family to good oral health is to lead by example. Start by making dental care part of your family’s daily routine. Depending on the age of your child, there are a number of methods that parents can try to encourage good oral health habits. Some suggestions for making toothbrushing less of a battle with young children include allowing your child to pick out his or her toothbrush (a variety of styles, including musical models, are designed to appeal to kids); letting your child see you and other family members brushing their teeth; and allowing your child to brush his or her own teeth. Parents should supervise toothbrushing by children younger than age 8 to make sure they are doing a thorough job. Parents also should assist young children with daily flossing.

Do you have questions about your family’s oral health care? Be sure to talk to your dentist.

The Effects of Hormones

What do hormones have to do with a woman’s oral health?

Hormonal changes occur throughout a woman’s life, and with these hormonal changes come changes in oral health. Puberty, menstruation, pregnancy, and menopause all can have an effect on a woman’s oral health. The use of oral contraceptives (birth control pills) can affect a woman’s oral health as well.

When might I notice changes in my oral health?

During puberty, fluctuations in hormones can make gums more susceptible to gingivitis. As a result, the gums may appear red and swollen, and they can bleed.

During menstruation, women who have a tendency to develop canker sores and cold sores may develop a pattern in which these sores recur during every menstrual cycle.

During pregnancy, gingivitis may develop. In fact, gingivitis is the most common oral condition associated with being pregnant. Sometimes, however, women will avoid dental checkups for fear that treatment might harm the developing baby. In truth, untreated gum infections and decayed teeth can put a mother and her baby at risk. Dental infections may be responsible for as much as 5 percent of low-birth-weight pre-term babies. Pregnant women need routine checkups. Always tell your dentist and his or her staff if you are pregnant.

Some women also experience dry mouth while pregnant. Taking frequent sips of water and chewing sugarless gum or candy can help alleviate this symptom. Women who experience morning sickness need to brush their teeth more frequently than twice a day. This will help to prevent stomach acids from contacting the teeth and causing permanent damage to tooth enamel.

The use of oral contraceptives may cause gum tissue changes in some women. Women who use birth control pills may also be more prone to healing problems or dry socket after tooth extraction.

During menopause, women may experience oral changes that include pain, a burning sensation in the oral tissue, changes in taste, and dry mouth.

After menopause, there is an increased risk of developing osteoporosis, which may increase the chance of tooth loss.

How can I maintain good oral health throughout my life?

Brush twice daily with toothpaste containing fluoride and floss once daily. Have your teeth professionally cleaned and examined by your dentist every six months (or more frequently if recommended by your dentist). Eat a well-balanced diet, and always tell your dentist and his/her staff about any medications, vitamins, and supplements that you are taking.

Ask your dentist any questions that you have about your oral health. Together, you and your dentist can create a treatment and prevention plan that specifically meets your needs. For more information about women’s oral health, visit www.knowyourteeth.com

A Minimally Invasive Procedure for Denture Stabilization

It is not widely known that over 25% of adults in the US over 40 years old wear dentures or partial dentures. Of these many hundreds of thousands suffer from ill-fitting, loose and floppy dentures. Not only is this very embarrassing in public or in front of friends and family, but it can be very harmful to your health. Here’s why:

Not being able to properly chew and digest your food leading to a multitude of health related consequences.

Digestive disorders and healthy regular bowel activity are effected.

Loose dentures that constantly rub and irritate the gums can potentially lead to oral cancer.

The emotional problems related to this looseness are very real and cannot be underestimated.

Ones personal appearance can be a source of embarrassment and loss of self-image.

</ br>

WE HAVE A SOLUTION – MINI IMPLANTS!

</ br>

Denture Implant Image – Indianapolis Dentist – Dr Jerrold Goldsmith

Not widely known, there is now a predictable, safe, painless, affordable, and approved method of STABILIZING your loose dentures in just one day!

This dental procedure is known as a Mini-Implant. Very small titanium implants are inserted into your toothless ridge. At the top of these implants is a small ball like extension. This ball will slightly protrude from your gums. Inside of your dentures are placed a snap-like clip that engages the ball structure firmly, thereby reducing the movement and play experienced by millions of people.

Your dentist will determine whether any health issues you have could compromise your ability to have the procedure. Also, your dentist will evaluate your current gum condition and the condition of the denture you have. In some cases you may need a new denture or a reline of the existing one.

The Mini-Implant could be one of the very best health decisions you can make for yourself.
