• SINGLE PLAN: Only $179.00
• FAMILY PLAN: Only $179.00 for Head of Household
• Only $139.00 Each Family Member(s) in the Residence over 10 Years Old
• Only $79.00 Each Family Member(s) in the Residence 10 Years Old and Under
